The 2016 Sonoma County Community Health Needs Assessment is a large-scale look at the health of our community as developed collaboratively by local health care providers, health experts, county leaders and residents. The report informs and engages local decision makers, key stakeholders, and the community at-large in efforts to improve the health and well being of all Sonoma County residents.
Prioritized needs identified by the assessment include:
- Early childhood development
- Access to education
- Economic and housing security
- Oral health
- Access to healthcare
- Mental health
- Obesity and diabetes
- Substance use
- Violence and unintentional injury
